The College of Nursing is seeking a dedicated and experienced Senior Case Manager to join our Case Management Department supporting home and community-based waiver programs. This position plays a critical role in coordinating services for aging and disabled populations while helping individuals maintain independence and quality of life within their homes and communities. The Senior Case Manager will manage a full-time caseload and serve as an experienced resource within the department, providing guidance, support, and leadership to team members while ensuring high-quality case management services are delivered in accordance with organizational standards and program requirements.

This position is ideal for a compassionate professional who is passionate about advocacy, care coordination, and improving access to healthcare and community resources. The successful candidate will work independently under limited supervision and will be responsible for all aspects of the case management process, including admissions, assessments and reassessments, service planning, monitoring, coordination of care, and discharge planning. The role requires strong critical thinking, communication, and organizational skills, as well as the ability to develop meaningful relationships with clients, caregivers, providers, and interdisciplinary teams. The preferred candidate will have one year paid professional experience with the aging adult or disabled population.
