Location: Will be responsible for delivering member-facing nursing services and care coordination in the following counties - Bell, Bosque, Burleson, Comanche, Erath, Gillespie, Grimes, Lampasas, Llano, McLennan, Mills, San Saba, Washington Counties

SHIFT: Mon - Fri 8am - 5PM CST

Duration: 12 months, possible to Extend

Description: Job Profile Summary: In this role, day-to-day responsibilities are centered on direct member care coordination, field visits, and compliance-driven assessments within the STAR+PLUS LTSS Waiver program: conducting in-home or community-based visits for annual reassessment, bi-annual reassessments, and change in condition visits; completing comprehensive clinical and functional assessments, developing person-centered service plans, coordinating with PCPs, specialists, agencies, etc.; addressing care gaps and urgent needs, facilitating service authorizations and referrals, providing member and caregiver education, completing real-time, audit-compliant documentation, and escalation and complaints management.

Visits are completed within the required timeframes and frequency, timely closures of worklist items and follow-ups, audit-ready and accurate documentation, appropriate development and updates of service plans, identification and closure of care gaps, reduction in safety risks or service delays, and effective coordination of services.

This role is unique due to the requirement of in-person engagement, supporting of the member with both medical conditions and social determinants of health, compliance to state-mandated assessments, and traveling.

Candidate Requirements
Education/Certification	Required: Graduate from an Accredited School of Nursing			Preferred: Bachelors degree in Nursing

Years of experience required: 4-6 years of related experience Disqualifiers: Less than 4-6 years experience Additional qualities to look for: Experience with other MCOs, home health nursing, or case management
