Summary:
Location: Must live in or around the area of Bryan, College Station, Waco, Killeen, Temple, or Gatesville.
Work Mode: Initially remote, with field visits required
Duration: 6 months, possibility of assignment extension or conversion
Shift: Monday – Friday (8am - 5pm CST)
Responsibilities:
Coordinate services for members, including setting up and coordinating appointments, services, supplies, and equipment.
Conduct 3-4 in-home visits per week to complete the Waiver Upgrade Assessment.
Complete documentation and assist in coordinating services for members.
Interact with team members and other departments via telephone, Microsoft Teams, email, and video.
Participate in remote training, with some in-person training required for field visits.
Requirements:
Graduate from an Accredited School of Nursing or a Bachelor's degree.
Registered Nurse, TX license.
Reliable transportation and driver's license.
Reliable internet connection.
Preferred Skills:
PDPM Certification.
Minimum 4 years of experience working with the adult Aged, Blind, or Disabled population.
Prior experience as a Care Manager or Service Coordinator for a Managed Care Organization, or similar.
