    Job Description

    SUMMARY

    Responsible for leading facility relations, community outreach, and stakeholder engagement initiatives throughout Wisconsin to support the effective delivery of Non-Emergency Medical Transportation (NEMT) services. Develops and maintains strong partnerships with healthcare facilities, clinics, community organizations, and other stakeholders to improve care coordination, enhance member access to transportation services, reduce service barriers, and improve overall outcomes. Provides education, training, and ongoing support to facilities and community partners while serving as a liaison between stakeholders and internal departments to promote service excellence and operational effectiveness.

    Position contingent upon award of contract.

    ESSENTIAL FUNCTIONS:

    • Build and maintain strong relationships with healthcare facilities, clinics, community organizations, healthcare professionals, and other key stakeholders to support effective NEMT service delivery.
    • Conduct on-site visits, meetings, and outreach activities with healthcare facilities and community partners to provide education, training, and support regarding NEMT services and processes.
    • Serve as an advocate for NEMT services within the community, promoting awareness, access, and utilization of available transportation resources.
    • Educate healthcare providers, facility staff, members, and community partners on transportation services, scheduling procedures, standing orders, discharge coordination, service expectations, and escalation processes.
    • Coordinate member education initiatives, including travel training, public transit resources, and other transportation support programs designed to improve access to care.
    • Respond to and resolve facility- and member-related transportation concerns by collaborating with Operations, Quality Assurance, and other internal departments to identify solutions and improve service outcomes.
    • Identify recurring service issues, operational barriers, and stakeholder concerns; recommend and coordinate corrective actions and process improvements.
    • Support Department-requested outreach events, community presentations, stakeholder meetings, training sessions, and communication initiatives.
    • Act as a liaison between healthcare facilities, community partners, members, and internal departments to ensure effective communication and coordination of transportation services.
    • Prepare and deliver presentations, training materials, and educational resources to healthcare providers, community organizations, and stakeholder groups.
    • Monitor outreach activities and stakeholder feedback to identify opportunities for service enhancements, increased engagement, and improved member outcomes.
    • Maintain accurate records of outreach activities, training sessions, stakeholder interactions, and issue resolution efforts.
    • Perform other duties as assigned.

    QUALIFICATIONS REQUIRED

    • Effective communicator with proven leadership skills and abilities.
    • Ability to analyze, organize, prioritize, implement, and control business activities.
    • Clinical, case management and/or discharge experience a plus.
    • High level of technical competence to include proficiency with Microsoft Office Suite.
    • Excellent problem-solving skills: must be able to develop and implement action plans to address issues.
    • Ability to travel throughout assigned region / territory.
    • Approximately 20% of work time is conducted in an office setting and approximately 80% will require travel.
    • Physical and Mental Requirements: - Strong problem resolution skills. - Must have a proactive approach to relationship building. - Must be solutions oriented, creative innovative thinker. - Must be able to work independently and as a team member.

    MINIMUM REQUIRED EDUCATION:

    MSW preferred; bachelor's degree in business from an accredited college or university preferred or commensurate experience in similar capacity.

    Experience in healthcare outreach, provider relations, training or card coordination

    Ability to travel statewide
