Project and Program Leadership
Mammogram Bus Program Leadership
Grant Management
Preventive Health Programs
Develop and oversee preventive health initiatives focused on improving population health outcomes.
Support programs related to:
Colorectal cancer screening
Diabetes prevention and management
Hypertension control
Vaccination and immunization campaigns
Tobacco cessation
Nutrition and wellness education
Social determinants of health interventions
Monitor care gap closure efforts and preventive service utilization.
Collaborate with clinical, operational, and community teams to increase patient engagement in preventive healthcare services.
Develop strategies to reach vulnerable, underserved, and high-risk population
Quality & Value-Based Care
Support organizational quality improvement and value-based care initiatives.
Monitor performance related to:
Medicare Stars
Medicaid quality programs
Accountable Care Organization (ACO) metrics
Preventive screening measures
Chronic disease management outcomes
Analyze performance data and develop action plans to improve quality metrics.
Collaborate with providers, care teams, and operational leaders to address care gaps and improve patient outcomes.
Support regulatory compliance and accreditation activities related to quality and population health.
Lead performance improvement initiatives that drive quality outcomes and financial sustainability.
Executive & Cross Departmental Support
Serve as a strategic advisor and resource to executive leadership regarding population health initiatives and community health programs.
Prepare executive reports, presentations, dashboards, and performance summaries.
Facilitate collaboration across departments including:
Population Health
Finance
Quality Management
Marketing and Community Relations
Information Technology
Human Resources
Participate in organizational committees, task forces, and strategic planning efforts.
Support executive leadership with data analysis, project planning, and special initiatives.
Assist in developing policies, procedures, and operational standards related to population health programs.

POSITION SUMMARY:
The Senior Manager, Population Health Projects, Programs & Grants is responsible for the strategic leadership, development, implementation, and oversight of population health initiatives, grant-funded programs, preventive health services, and quality improvement projects designed to improve patient outcomes and support organizational growth. This role serves as a key leader in advancing population health strategies, value-based care initiatives, community outreach programs, and health equity efforts.
The Senior Manager collaborates with executive leadership, clinical operations, community partners, and external stakeholders to drive innovative programs, secure funding opportunities, manage grants, and ensure successful execution of projects that align with the organization's mission and strategic objectives.
