Position summary: Primarily responsible for coordinating the implementation, evaluation, and expansion activities of the Healthy in the City program, a family-centered health initiative designed to prevent childhood obesity and improve health outcomes among children and caregivers. Reports to the Manager of Community Programs and plays a key role in supporting implementation of program priorities, coordinating partnerships, facilitating operational activities, and advancing initiative goals. Key responsibilities:
Coordinate implementation of the Healthy in the City program expansion, supporting achievement of program goals through management of project plans, timelines, operational processes, and stakeholder communications. Work closely with the Manager of Community Programs to ensure activities are aligned with strategic priorities and funding requirements.
Support delivery and continuous improvement of family-centered health programming, including nutrition education, physical activity initiatives, family engagement activities, resource navigation, and community-based wellness opportunities designed to promote healthy lifestyles and reduce risk factors associated with childhood obesity.
Coordinate relationships and activities across community health centers, healthcare providers, community-based organizations, schools, recreational partners, and other stakeholders to support program implementation, participant engagement, and access to resources.
Facilitate collaboration among program, clinical, and community partners to promote effective communication, resolve implementation challenges, and ensure a coordinated approach to serving children and families.
Serve as a liaison between program operations and evaluation efforts, coordinating with health center staff and evaluation teams to support collection of participant, family, clinical, and program data. Help establish and maintain processes that ensure accurate, timely, and consistent reporting of outcomes, participation metrics, and other data needed for program evaluation, quality improvement, and grant reporting.
Monitor program performance and implementation progress through regular review of activities, outcomes, and operational metrics. Identify challenges and opportunities for improvement and contribute to the development and implementation of solutions that strengthen program effectiveness and participant experience.
Coordinate program meetings, trainings, stakeholder convenings, and partner communications, and assist in the preparation of reports, presentations, funding updates, and other materials for leadership, funders, and community partners.
Support planning, documentation, and implementation of program growth and expansion efforts, including development of operational workflows, partnership structures, and implementation resources that facilitate replication of the model at additional community health centers.
Minimum qualifications Education:
An Associate's degree or equivalent experience in Public Health, Community Health, Health Administration, Social Work, Human Services, Population Health, or a related field is required. A Master’s degree is preferred.
A min of 5 years of experience in program coordination, public health, community health, healthcare administration, population health, or a related field is required. Experience coordinating complex initiatives involving multiple stakeholders, community partnerships, and program evaluation activities is preferred.
