Position Summary:
We are currently searching for a Senior Social Service Representative who will provide assistance to patients including special populations as assigned. The Senior Social Service Representative will also provide general and specialized screenings, assessments, advocacy, and referrals to internal and external services as needed. This role supports needs of patients in the lobby of El Nuevo San Juan while taking on support of UHP's partnership with external firm that provides patients with legal services.
Minimum Qualifications:
- High School Diploma required.
- Previous experience working in a community-based health center or similar setting.
- Bilingual Spanish/English required.
- 2 - 5 years of related experience providing support services, health advocacy, or related field required.
- Proficiency with computers and software.
- Strong communication skills.
- Ability to multi-task.
Key Duties:
- Provides case management assistance to patients including populations as assigned.
- Acts as a liaison to support patients seeking legal guidance with attorneys.
- Provides letters to patients including letters for public assistance, housing, transportation, IRS, schools.
- Provides community-based resources to the population based on site/ department/ program need.
- Advocates on patient's behalf to obtain specific resources or services including specific or specialized services.
- Works closely with departmental coordinators and team members to provide patients with support.
Work Environment Type, Location, and Schedule:
The Bronx, Hybrid, 4 Days onsite, 1 day remote, Monday to Friday, 8:00am - 5:00pm or 9:00 am-6:00 pm
Starting Compensation: Minimum: $21.90 Hourly; Maximum: $21.90 Hourly ($45,560 based on 40 hrs/week)
