Job Description

We are seeking a Senior Social Worker to join our team serving veterans at the Arizona Department of Veterans' Services. The ideal candidate will have strong experience in supervising social services programs, case management, and regulatory compliance and a proven ability to lead teams, ensure timely and accurate documentation, and deliver person-centered services that improve quality of life.

Responsibilities:

• Plan, organize, and supervise the social services program of the Arizona State Veteran Home.
• Ensure timely completion of assessments and documentation per federal and state regulations and facility policies and procedures.
• Ensure compliance with the PASRR process and monitor Medicare notices for timely delivery to beneficiaries.
• Supervise staff to meet unit goals, including planning, assigning, reviewing work, and conducting monthly 1:1 coaching and annual appraisals.
• Address staff issues and recommend disciplinary actions as needed, including interviewing and selecting staff.
• Oversee clinically appropriate discharge planning and ensure effective communication to stakeholders.
• Complete consults and referrals to Veterans Administration specialists as indicated.
• Partner with residents, families, and staff to uphold Resident's Rights.
• Assist with department budgeting, work within parameters, and monitor expenditures.
• Maintain a personal caseload and participate in facility meetings and committees.
• Support continuous improvement through AMS participation, training, and team collaboration.

Required Skills & Qualifications:

• Master's degree in Social Work.
• Active social work license issued by the Arizona Board of Behavioral Health Examiners upon placement.
• One year of supervisory experience in a health care setting.
• Experience in long-term care and knowledge of CMS and State regulatory standards.
• Proficiency in case management, counseling, resident assessment, and care plan development.
• Strong verbal and written communication, analytical ability, and problem solving.
• Ability to supervise and manage teams and maintain confidentiality.

Preferred Skills:

• Knowledge of geriatric psychiatric issues, disease processes, and side effects of psychotropic medications.
• Familiarity with physical and chemical restraints, community resources, and governmental programs affecting clientele.
• Understanding of long-term care billing standards and the MDS system.

Pre-Employment Requirements:

• Ability to obtain and retain a valid Arizona DPS Fingerprint Clearance Card.
• Current negative TB skin test (Mantoux) within the last 6 months or 1 year, as applicable.
