General Description

The Service Coordinator is a valuable member of our Prevention Team who is within Child and Family Connections and Prevention. Under general supervision, performs and offers direct individual and community services, client advocacy, and provides both general and specific health/developmental intervention connections in the community and natural environment settings. Work assignments are in-depth and specialized with specific areas of responsibility and independence of action.

Schedule Hours: 37.5 hours a week

  • Monday - Friday (8:00am - 4:30pm)

Essential Functions

  • Provide an intake for new referrals including an effective description of the Illinois Early Intervention System to families including, but not limited to, family rights, procedural safeguards, availability of all Early Intervention services and all aspects of the Early Intervention system within program timelines.
  • Facilitate assembly of appropriate multidisciplinary teams utilizing community providers, providing for a family choice of providers, for the purposes of eligibility determination, Individual Family Service Plan (IFSP) development, IFSP reviews and Exit meetings utilizing the guidelines of the Early Intervention System.
  • Request and obtain pertinent medical records, evaluation/assessment reports and other information relevant to the Early Intervention System for all clients including acquisition of appropriate prescriptions, releases of information and family signatures on program forms.
  • Maintain organized client records meeting percentage of proficiency required by the program, utilizing local office and state Early Intervention format and timelines.
  • Conduct developmental screenings at intakes for all children at or above three months old and collaborate with Social/Emotional Specialist for each result.
  • Consistently utilize program provided laptop computer during all scheduled meetings with families to enter required data during meetings.
  • Expedite all referrals for evaluations, assessments and direct care services for families within the state imposed 45 day timeline.
  • Maintain monthly contact with each caseload family as shown by documentation in the state provided computer system for the purposes of monitoring family linkages with service providers, community resources and to adjust EI records due to family changes.
  • Maintain a caseload of 60 plus families completing all intakes, IFSP meetings, six-month reviews, change meetings, transition meetings and exit meetings on identified timelines and meeting the program identified standards with stats each month.
  • Adhere to program set timelines, including but not limited to reports and schedules.
  • Initiate contact with all new referrals within five working days of initial assignment.
  • Effectively track your Early Intervention credentialing needs and maintain an active Service Coordination credential.
  • Consistently provide timely transition referrals (2.6 years or earlier) for caseload children following established program procedures including referral to Parent Liaison if family declines transition.
  • Consistently ensure that each child on your caseload has a transition meeting at least 90 days prior to age three.
  • Consistently connect families to needed Early Intervention services while maintaining a monthly wait list not exceeding 3% of your caseload.
  • Promptly return all phone call messages, hand written messages, e-mails, faxes and other forms of communication from families, providers, program staff and others consistently within 48 business hours of their arrival.
  • Support the training of new program staff as assigned.
  • Perform other duties as assigned.

Knowledge Skills Abilities and Education Required

  • Bachelors degree, in a related field (health education, social work, psychology, sociology, early childhood development, communications, etc.)
  • Case management experience preferred.
  • Requires a valid Illinois driver's license and automobile insurance.
