Phoebe is seeking a compassionate, organized, and service-oriented Service Coordinator to support residents by connecting them with the resources and services they need to maintain their independence and quality of life. This role provides case management, conducts assessments, coordinates community resources, and serves as an advocate for residents and their families.

Responsibilities
- Conduct psychosocial and needs assessments for residents.
- Develop individualized service plans based on each resident's health, social, and emotional needs.
- Provide case management, crisis intervention, and short-term supportive counseling.
- Coordinate referrals to community resources and monitor service delivery.
- Educate residents about available programs and assist with application processes.
- Coordinate services such as transportation, home health, Meals on Wheels, homemaker services, financial assistance, counseling, preventative health programs, and other community resources.
- Establish and maintain strong relationships with community agencies and service providers.
- Maintain accurate documentation and resident records.
- Collaborate with interdisciplinary teams to support resident well-being.
Qualifications
- Bachelor's degree in Social Work or a related human services field required.
- Minimum of one (1) year of experience in social work, case management, or providing services to older adults required.
- Experience working with the geriatric population strongly preferred.
- Knowledge of community resources, supportive services, and healthcare systems.
- Understanding of hospital admission and discharge planning preferred.
- Excellent communication, interpersonal, and organizational skills.
- Ability to manage multiple priorities while maintaining attention to detail.
- Proficiency with Microsoft Office and electronic documentation systems.
- Bilingual (English/Spanish) or fluency in Spanish is preferred but not required.
