Our mission is to guide brain injury survivors, individuals with disabilities, and their families, by ‘bridging the gap’ between surviving and living lives of independence with dignity and hope. Our organization assists people to rebuild skills they were once able to do independently before terrible tragedy struck in the form of a car accident, stroke, assault, fall, or before they were struck with a debilitating disease.
Here's What We’re Looking for in a Service Coordinator:
The Service Coordinator plays a critical role in helping brain injury survivors and people with disabilities to re-acclimate to a home setting from a nursing home. The SC provides linkage and referrals to community services and resources that will help to maximize the person’s independence and support their health needs throughout the Greater Rochester/Finger Lakes Region.

Organization: Life is always changing, and this is often the case with the people you will be working with on your caseload. No day will ever be the same. Expect to jump from task to task depending on the needs of your caseload and upcoming deadlines. To do this well, we will need you to be great at planning, prioritizing, multi-tasking and doing what it takes to get stuff done!

The pay range for this role is $21 to $23 per hour. Salary offers are commensurate with education and experience.
