Thrive By 5 is currently hiring Service Coordinators for the Early Intervention Program in New York City. This position involves providing continuous and interactive services that ensures the successful participation of eligible children aged 0-3 and their families in a home-based and/or community-setting. Opportunities are available in Manhattan, Queens, Brooklyn, Staten Island, and the Bronx.
Essential Duties and Responsibilities:
The essential duties and responsibilities are not limited to the following:
Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
Ensure the IFSP outcomes and strategies reflect the family's priorities, concerns, and resources; and that changes are made as the families/child's needs change.
Coordinate the provision of all services that the child needs and/or are receiving.
Facilitate the timely delivery of services.
Continuously seek appropriate services and situations that benefit the development of the eligible child for the duration of the child's eligibility.
Perform related duties as required.
Qualifications, Knowledge and Experience:
Bachelor's Degree in health and/ or human services field.
Bilingual preferred
Ability to effectively communicate with clients and families via phone and email.
