About the job:
The Service Coordinator provides general case management (including intake) and referral services to all residents needing such assistance. Formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services, and periodic reassessment of resident’s situation and needs) for a resident is needed when such service is unavailable through the general community.

The requirements listed below represent the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
Bachelor’s Degree-concentration in Social Work, Gerontology, Psychology or Public Health.
Training in the aging process, elder services, disability services, drug and alcohol abuse and mental health issues.
Proven experience in service management, including organizing, problem solving, and advocating.
Knowledge of community services in the region, particularly with services that are provided for the population living within the facility.
Knowledge of computer operations, to include basic data entry.
Ability to communicate effectively and maintain good relations with residents and employees.
Aware of eligibility for and procedures of federal and state entitlement programs.
