Job Description:

We are seeking a dedicated, engaging, and highly organized Service Coordinator to support residents across two senior communities. This position requires the ability to travel between both properties.

Bilingual preferred.

Work Schedule and Locations- Part Time 20 hours a week

Kachina Place

Monday and Friday, 9:00 AM - 2:00 PM

Position Summary

The Service Coordinator is responsible for coordinating supportive services, enrichment activities, and community resources that help residents maintain independence and a high quality of life.

Education

  • Bachelor's degree in social work or a related field preferred.

Experience

  • Two or more years of experience in social service delivery, ideally within senior or family populations.

  • Strong working knowledge of community resources and supportive services.

  • Demonstrated ability to advocate, organize, problem-solve, and deliver consistent results.

Key Responsibilities

  • Coordinate programs, services, and activities for residents.

  • Provide support across both communities while following the assigned weekly schedule.

  • Assist residents with applications for support programs such as Medicaid, Medicare, and other benefit programs.

  • Organize and lead social events, engagement activities, and educational opportunities.

  • Engage residents daily to identify needs and provide appropriate referrals.

  • Serve as a liaison to community agencies and service providers, building a strong network of partnerships.

  • Identify affordable service providers and negotiate discounts when possible.

  • Develop and maintain a Resource Directory of state and local agencies.

  • Offer educational sessions on health, wellness, life skills, and available support services.

  • Deliver excellent customer service by supporting residents with basic needs and concerns.

  • Assist residents in accessing services such as transportation, meals, personal support, financial assistance, and more.

  • Document and report resident or family concerns as required.

  • Maintain positive and professional relationships internally and externally.
