Job Description

Medicaid Waiver Service Facilitator/Case Manager (Independent Contractor – 1099) - Hampton Roads/Tidewater Virginia and surrounding areas.

The Service Facilitator/Case Manager (SF) is responsible for coordinating and monitoring Medicaid Home and Community-Based Waiver services to help individuals safely remain in their homes and communities. This position is in Hampton Roads/Tidewater and the surrounding areas. The SF will conduct monthly in-person, in-home visits for initial intake assessments to develop or update person-centered Plans of Care, monthly routine follow-up visits, and annual or biannual reassessment visitsin compliance with Virginia DMAS regulations.

The role requires maintaining accurate documentation; strong organizational, communication, and time-management skills are essential, as this position requires working independently and managing a flexible schedule. Must have reliable transportation and the ability to travel throughout the surrounding counties.

The ideal candidate will serve as an advocate and resource for the individual and their family. We support caregivers and families so their loved one can remain safe at home, while promoting independence, choice, community integration, health, safety, and achievement of identified outcomes.

Job Requirements & Education:
  • High school diploma or GED required.
  • Experience working with individuals with chronic health issues, intellectual/developmental disabilities, or mental health disabilities required.
  • Successful completion of Virginia DMAS Service Facilitation Module Training (before providing services).
  • Knowledge of Virginia Medicaid Waiver programs and person-centered planning.
  • Excellent documentation, computer, communication, and organizational skills.
  • Valid driver's license, reliable transportation.
  • Ability to pass all required background screenings.

Preferred:
  • Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation Services, Disability Services, Nursing, or a related field.
  • Experience working with individuals with intellectual or developmental disabilities.
  • Experience with Virginia Medicaid waiver programs and forms.
  • Experience with Consumer-Directed service and Fiscal Agent processes.
  • Knowledge of DMAS and DBHDS regulations and documentation requirements.
  • Experience with electronic health records or case management/documentation systems.
