Position Summary

The Service Manager provides case management and service coordination for individuals receiving Medicaid Waiver services while supervising a team of Direct Support Professionals (DSPs). This position serves as the primary contact for individuals, families, care coordinators, and community partners to ensure quality supports and successful implementation of Individual Service Plans (ISPs).

Key Responsibilities

  • Manage an assigned caseload and coordinate services to meet individual goals and needs.
  • Develop, implement, and monitor Individual Service Plans (ISPs).
  • Facilitate quarterly team meetings and monitor progress toward outcomes.
  • Connect individuals and families with community resources, healthcare providers, and support services.
  • Maintain accurate documentation, medication records, and HIPAA compliance.
  • Supervise, schedule, train, and support DSP staff.
  • Assist with recruitment, onboarding, and retention efforts.
  • Conduct regular quality assurance visits to ensure high-quality service delivery.
  • Provide direct support services as needed.
  • Maintain compliance with Medicaid Waiver requirements and agency policies.

Qualifications

Required

  • High School Diploma or GED.
  • At least 18 years of age.
  • Ability to pass and maintain required background checks.
  • Strong organizational, communication, and problem-solving skills.

Preferred

  • Experience in case management, healthcare, human services, or developmental disabilities.
  • Knowledge of Medicaid Waiver programs and person-centered planning.
  • Supervisory or leadership experience.

Work Environment

This position includes office, community, and client-based work. Local travel, occasional lifting, and direct support duties may be required.
