Job Summary
The Care Management Aide supports the care management team by assisting with care coordination, discharge planning, and administrative tasks. This role provides exposure to interdisciplinary healthcare processes while helping ensure smooth patient transitions and accurate documentation.
Minimum Qualifications
- Education: High school diploma or equivalent required
- Maryland resident
- Individuals eligible for the Maryland Service Year Program (generally ages 18-24)
- Experience: None required; customer service, volunteer, or healthcare exposure preferred
- Licensure/Certification: None required
- Skills & Abilities:
- Strong communication and organizational skills
- Ability to multitask and work in a team environment
- Basic computer skills and ability to learn EMR systems
- Interest in healthcare careers
- Attention to detail and follow-through
What You'll Do
- Assist with care coordination and patient tracking activities
- Participate in interdisciplinary rounds and observe care planning
- Help coordinate discharge plans, appointments, and follow-up services
- Communicate updates and barriers to care team members
- Enter and update non-clinical information in the EMR
- Support documentation, referrals, and administrative tasks
- Assist with tracking metrics and quality initiatives
