Site Medical Director – Primary Care

What You'll Do
As Site Medical Director, you will provide both clinical leadership and direct patient care.
Clinical Leadership
- Lead and supervise physicians, nurse practitioners, and physician assistants at the assigned health center.
- Foster a culture of clinical excellence, accountability, and continuous improvement.
- Monitor provider productivity, quality outcomes, and patient experience metrics.
- Coach and mentor providers through regular performance discussions and professional development.
- Partner with the Chief Medical Officer and interdisciplinary leadership to implement strategic clinical initiatives.
- Support Patient-Centered Medical Home (PCMH), quality improvement, population health, and regulatory initiatives.
- Participate in medical leadership meetings and organizational planning efforts.
Patient Care
- Maintain an active primary care practice while serving in a leadership capacity.
- Provide approximately 28 hours of direct patient care and 12 hours of administrative leadership each week.
- Diagnose, treat, and manage acute and chronic medical conditions across diverse patient populations.
- Coordinate care with behavioral health, nursing, pharmacy, and specialty services.
- Participate in after-hours call rotation and maintain hospital privileges as required.
- Support preventive care, chronic disease management, HIV prevention and treatment, gender-affirming care, and whole-person healthcare.
What We're Looking For
Required Qualifications
- MD or DO from an accredited medical school.
- Completion of an accredited Family Medicine residency program.
- Active Illinois Physician License (or ability to obtain).
- Current DEA registration.
- Board Certification (or ability to obtain within two years of hire).
- Minimum three years of clinical practice experience.
- Minimum three years of leadership or supervisory experience, preferably in an ambulatory or community health setting.
Preferred Qualifications
- Experience working in a Federally Qualified Health Center (FQHC) or other community-based healthcare organization.
- Experience leading quality improvement initiatives and developing clinical protocols.
- Demonstrated success mentoring and developing providers.
- Commitment to culturally responsive, trauma-informed, and affirming care.
- Experience serving diverse, medically underserved populations.
