Job Summary:


Provides direction and leadership to the members of the healthcare team to foster a supportive collaborative environment to meet the needs of the resident family physician and significant others in the delivery of nursing services. 100% of the incumbent's time will be spent at Bruceville Terrace. Position Scope: The incumbent is accountable for delivering high-quality nursing services on an assigned unit in a cost-effective environment and through effective human resource management. The incumbent works collaboratively with the interdisciplinary team to ensure a holistic approach to the provision of nursing services to the identified patient population. This position provides leadership while contributing to the long-range planning processes of the unit's day-to-day operational oversight risk management performance improvement and customer service. Direct reports: LVN Certified Nurse Assistants and Unit Assistants. Major Challenges: The major challenges for the incumbent include the active promotion of and support for the integration of services on the unit; to act as facilitator teacher and coach to the nursing team members enhancing their development as clinical experts and members of the interdisciplinary team and leaders in nursing; and to develop innovative methods in support of the facility performance improvement activities. Freedom to Act: This position is accountable for meeting the patient and family needs through skill mix evaluation staff assignments human resource development and management. The position has autonomous professional accountability for directing and coordinating the provision of services on the unit. The position accurately interprets information and coordinates resolution to prevent negative patient outcomes while ensuring fiscal responsibility and professional standards. This position has accountability and authority to make decisions regarding patient placement on the units based on the level of care requirements and ability to meet the patients needs. This position is responsible for recruitment recommendations on hiring /termination of staff conducting performance reviews coaching and administering corrective action. Decisions are submitted to the Director LTC/SNF Nursing regarding recommendations for educational programs and expenditures beyond a $500.00 limit.


Responsibilities:


+ Effectively directs nursing practice and supports the interdisciplinary team process.

+ Creates and maintains a professional environment that promotes recruitment and retention of qualified staff.

+ Plans, directs, and supervises nursing and healthcare services while maintaining an appropriate skill mix to provide quality care.

+ Defines job expectations and lines of authority as well as communicating standards of practice and serving as a clinical resource.

+ Collaborates effectively with key physicians, nursing, therapy, and other ancillary services to manage the care of thepatient in the most cost-effective and quality-driven manner.

+ Participates effectively in CQI projects.

+ Maintains statistical information as requested on unit activities and processes.

+ Monitors compliance with facility policies and procedures, OBRA, and Title 22 requirements.


Qualifications


Minimum Required:


+ Recent Skilled Nursing experience with 1 of the 3 years in a progressive leadership role.

+ Current California RN License

+ Current BLS certification (American Heart Association)

+ Gerontologial nursing practice, personnel management, CQI, state and federal regulations and JCAHO standards.

+ Nursing process, analysis of data, problem solving, effective communication, time management and inter-personal skills.

+ Ability to gather, analyze and facilitate services or resolution to meet the needs of our customers.

+ Ability to deal with difficult and/or sensitive issues or customers, handle emergencies expediently to maintain safety of environment or customers.

+ Strong computer and spreadsheet skills.

Bruceville Terrace has been providing quality compassionate care since 1989. Licensed as the 171-bed sub-acute skilled nursing facility of Methodist Hospital of Sacramento Bruceville Terrace offers care for the elderly as well as those requiring extended recoveries.


Bruceville Terrace provides skilled nursing rehabilitative and complex medical services to our patients and residents using an interdisciplinary team approach. Over half of the facility is dedicated to skilled nursing/convalescent care while the remaining beds are committed to meet the medical and rehabilitative needs of the short-term patient after a stay at the acute care hospital.
