Position Responsibilities:

• Conduct an in-person comprehensive speech therapy assessment as directed by IDT. • Coordinate with the interdisciplinary team to develop a comprehensive plan of care for each participant when speech therapy is needed. • Communicate progress, changes, and stated concerns to the interdisciplinary team. • Participate in the IDT and care plan meetings as requested. • Examine, test, diagnose, and administer remedial treatments for patients with speech and hearing disorders. • Initiates and/or completes patient assessment, following time frames established in departmental guidelines, to determine treatment required and completes documentation as observed by supervisor or during chart review. • Performs thorough assessment of patients functional conditions to set plan of treatment with appropriate short and long-term goals. • Updates patients progress and goals, as evidenced by appropriate documentation. • Documents charges with each initial evaluation and daily treatment as appropriate and as observed by supervisor and other department personnel. • Assess the need for adaptive durable medical equipment. • Modifies treatment approach based on patients reaction to it and reassesses outcome as observed by supervisor. • Evaluates patients regarding the application of a wide variety of therapeutic techniques for rehabilitation of speech, language, hearing, and oral motor disorders. • Records speech samples and conducts phonetic analyses of speech ability. • Documents test results and recommends remedial treatments. • Performs evaluations with special attention to receptive and expressive language skills, speech fluency, vocal and oral motor competence, articulation, and auditory skills. • Works in conjunction with Supervisor/Manager to agree on treatment utilizing corrective exercises and special equipment. • Trains Eldercare staff in speech therapy and in other areas of practice. • Supervises activities of support personnel as authorized by supervisor. • Participates in Quality Improvement projects. • Participates in the clinical education program (student program) as reported by the coordinator for clinical education. • Performs initial and annual competencies on direct care staff under supervision. • Know and adhere to philosophy and goals of Eldercare. • Act only within the scope of his or her authority to practice.

Qualifications:

• Current Florida State SLP License. • CPR certification. • ASHA Membership preferred. • Graduate from Masters Degree Program from an accredited University in accordance with state licensing requirements. • Successful completion of Certificate of Clinical Competency. • A minimum of 12 months of clinical work experience; working towards approved project or at least 15 CEHs per year in specialty area (Swallowing, Voice, Communication, Neuro, geriatrics, etc.) and a minimum of 1 year working with a frail or elderly population.
