LPN Long Term Care | St. John's, Rochester, NY

Shift: Days / Evenings / Nights Rotating
Schedule: Weekends, Holidays, On-Call, Callback, and Charge responsibilities as needed

Position Description

• Provide direct nursing care to approximately 8 10 geriatric long-term care residents under the supervision of an RN.
• Care for older adult residents with chronic medical conditions and long-term care needs, including wound care; no tracheostomy or acute-care patients are expected.
• Perform comprehensive nursing assessments, monitor changes in residents' conditions, and demonstrate strong critical-thinking and clinical judgment.
• Administer medications and treatments safely and accurately while following physician orders, facility policies, and established nursing standards.
• Provide wound care and monitor skin integrity, recognizing signs of infection, deterioration, or other complications and escalating concerns appropriately.
• Assist residents with activities of daily living while promoting dignity, independence, comfort, and quality of life.
• Collaborate closely with RNs, CNAs, physicians, therapists, and other members of the interdisciplinary care team.
• Provide direction and oversight to CNA colleagues, ensuring appropriate delegation, resident care, safety, and completion of assigned duties.
• Document assessments, medications, treatments, resident responses, and changes in condition accurately in the facility's medical record.
• Communicate effectively with residents, families, caregivers, and healthcare providers regarding resident needs and changes in condition.
• Respond appropriately to changes in resident status and initiate appropriate interventions within the LPN scope of practice.
• Work independently and prioritize care effectively in a long-term care environment with multiple residents and competing clinical needs.
• Participate in charge nurse responsibilities, weekend/holiday coverage, on-call, and callback assignments according to unit requirements.
• Maintain a safe, clean, and therapeutic environment while adhering to infection-control, fall-prevention, medication-safety, and resident-safety protocols.

Required Qualifications

• Active New York State LPN License
• Minimum 2 years of Long-Term Care experience
• BLS American Heart Association (AHA) only
• Strong geriatric nursing and medication-administration experience
• Wound-care experience preferred/required based on unit needs
• Ability to work rotating Days/Evenings/Nights
• Ability to work weekends, holidays, on-call, callback, and charge assignments as required
• Strong communication, organization, independence, and critical-thinking skills
