SNF Activities Aide - Full Time - Flex

Reporting to the Director of Activities, it is the responsibility of the Activities Aide to work as a team member along with the nursing, dietary, rehabilitation, social work and housekeeping staff members to provide daily activity options as well as assist with other non-clinical tasks as needed or assigned.

Roles and Responsibilities:

  • Complete initial assessments of new admissions within seven days of admission. Complete documentation for Swing Bed patients within two days of admission to Swing Bed status.
  • Assist in monthly calendar planning.
  • Assist in transporting residents to and from programs. Accompany and assist residents on scheduled outings.
  • Keep attendance records of resident participation including types of involvement, level of and responses to involvement including involvement in volunteer opportunities. Document resident behaviors and interventions used. Utilize documentation in electronic medical record.
  • Post scheduled activities for the day and deliver newspapers to residents. Coordinate resident mail delivery as needed.
  • Report noted changes, need for information, etc. to other departments as warranted regarding resident status and program changes.
  • Participates required annual in-service training programs as assigned.
  • Maintain organization of supply cupboards. Set up activity area, including moving furniture, as needed for the various programs. Assist residents in obtaining materials as needed in completing projects for independent activities (i.e. books from the book cart).
  • Conduct group programs designed to promote cognitive and physical functioning, socialization, recreation, creativeness and spiritual expression.

Required Skills and Experience:

  • High School Diploma or GED

Preferred Skills and Experience:

  • Certified Nursing Assistant license
  • Previous long term care experience preferred
  • Preferred experience with senior care recreational planning
  • Computer Skills

Physical Requirements:

  • The responsibilities of this position involve significant physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling and twisting. Purpose and Essential Functions
