JOB SUMMARY: Serves as a clinical resource to SNF, clinics and acute care units on issues pertaining to patients with wounds, pressure ulcers, - fistulas, incontinence and diabetes. Provides direct patient care for assessment and education purposes. Establishes and maintains practice standards and provides ongoing staff education. Can provide diabetic or wound education - for patients and their support systems.
PERFORMANCE DIMENSIONS AND TASKS
Essential Function
Develops Protocols & Practice Standards for managing patients with wound and incontinence care needs.
Monitor patient outcomes on an ongoing basis.
Serves as a resource person for the Wound patient care practices.
Works with the Clinical Educator on the development of education programs specific to Wounds, and skin care.
Provides educational programs to agency staff and community groups.
Participates in Multi-disciplinary patient centered case conferences, as appropriate.
Maintains data and statistical reports as requested.
Participates in research projects related to WC issues.
Participates in Qualitydf Improvement.
Acts as a resource person to nursing staff.
Uses a customer-focused approach when collaborating with Agency customers including co-workers, patients, physicians, and staff from community agencies and institutions.
Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service education programs.
Educates staff, patients, and support systems on caring for diabetic disease model and healthy living as it pertains to their disease model.
Demonstrates the knowledge and skill necessary to provide care appropriate to the age of the patients served on his/her assigned visit.
Supports the Corporate Compliance Program by adhering to the Policies, Procedures & Department Manuals of the organization. Employee will attend all mandatory in-services and annual training programs relating to Corporate Compliance. Assures that activities the employee is responsible for comply with any federal and state regulations. Reports and corrects any such conflicts which may arise.
Collaborates with other caregivers to ensure that the patient receives information about services in the community or other entities that may assist them in managing their wounds and any co-morbidities.
Collaborates with the dietician to provide diabetics and support systems with nutritional needs to maintain a lifestyle that will enhance their ability to manage their diabetes.
Performs other duties as assigned.
