Compensation: $25/hour

Schedule: Monday to Friday (Remote)

TAG MedStaffing is hiring a Social Care Coordinator (Bilingual) in New York, NY for our client. This role will support individuals with care coordination, community resource referrals, service navigation, and ongoing engagement. The Social Care Coordinator will connect individuals with appropriate resources, maintain accurate documentation, and provide timely follow-up to support access to services.

Responsibilities as the Social Care Coordinator:

• Member Coordination: Schedule non-clinical assessments and provide ongoing support to members throughout program participation.
• Care Navigation: Connect members with appropriate healthcare, nutrition, and community-based social services based on identified needs.
• Referral Management: Coordinate referrals with internal teams and external service providers while tracking completion and member follow-up.
• Member Engagement: Manage inbound and outbound communications to answer questions, provide program guidance, and maintain member engagement.
• Documentation: Maintain accurate member records, referral activity, service updates, and required documentation within care management systems.
• Compliance: Follow applicable Medicaid, HIPAA, privacy, and healthcare documentation requirements.

Qualifications for the Social Care Coordinator:

• Education: High school diploma or equivalent is required, with an associate or bachelor's degree in Human Services, Healthcare, Social Work, or a related field preferred.
• Bilingual: Fluency in English and Spanish, including professional reading and writing proficiency, is required.
• Experience: 3+ years of customer service, member engagement, patient support, care coordination, or related experience is required.
• Industry Knowledge: Knowledge of Medicaid programs, social care services, healthcare navigation, or community-based resources is required.
• Technical Skills: Experience with Five9 is required, along with proficiency using CRM, care management, or similar technology platforms.
