As part of our commitment to developing the prevention offer at the front door, we are recruiting a part-time Social Care Coordinator to work within the countywide Prevention & Access Team. This role could be to cover the Scarborough area. If you are an experienced worker looking for a new challenge, we hope you will want to join us.
As part of the Prevention & Access Team you will work closely with our social workers, wellbeing workers and occupational therapists towards delivering a preventative offer to the residents of North Yorkshire to help reduce or delay the need for formal care and support services.
You will be required to work at pace in a fast-moving team environment, and this will involve undertaking a range of assessments and delivering simple solutions to enable people to maximise their independence.
You will determine service eligibility for adults with care and support needs against the national standards and ensure people and carers are safe from harm. Where harm or abuse has been identified, you will support the team to progress Safeguarding work.
We ensure that our social care coordinators have manageable workloads, supportive and transparent management and the opportunity for continuous professional development.
