Social Care Manager
Remote | Contract Opportunity | New York State | Pay: $23-24/hr.

Our client is seeking a Social Care Manager to support members with ongoing Health-Related Social Needs (HRSNs) through care coordination, eligibility assessments, and ongoing support services. This fully remote contract opportunity is ideal for professionals passionate about health equity, Medicaid populations, and helping individuals connect to critical community resources.

Responsibilities

  • Manage incoming referrals for social care and care management services
  • Conduct outreach, screenings, and eligibility assessments for Medicaid members
  • Develop individualized care plans based on identified needs
  • Coordinate referrals to housing, food access, transportation, and additional community resources
  • Monitor member progress and update care plans throughout service periods
  • Ensure timely follow-up with providers, members, and community partners
  • Maintain accurate documentation, case notes, and status updates
  • Collaborate cross-functionally to support continuity of care and positive member outcomes
  • Track referral trends, service utilization, and care management activity
Qualifications
  • 5+ years of experience in care management, case management, social services, healthcare, managed care, or nonprofit environments preferred
  • Strong understanding of Medicaid, social determinants of health, community resources, or health equity initiatives
  • Experience managing caseloads in a high-volume environment
  • Excellent communication, documentation, and organizational skills
  • Comfortable navigating multiple systems and technology platforms
  • Associate degree preferred; equivalent relevant experience considered
