The Social Service Coordinator is responsible for overseeing, coordinating and implementing the community-based social services and programs at Kin On. ROLES AND RESPONSIBILITIES: - Support and advocate for Kin On mission, vision and core values
- Coordinate and participate in community outreach to educate and inform the community on issues such as emotional health, Alzheimer's disease and dementia, Medicare and Medicaid, long-term care services.
- Conduct workshops and presentations to address mental health, caregiving, self-care, chronic disease self-management, fall prevention and related issues.
- Provide information and assistance, referral, service coordination and consultation services to clients.
- Provide consultation to help family caregivers to cope with their caregiving needs.
- Conduct client interview and assessment to determine the nature of client's situation and needs by gathering the client's medical, psychological, emotional and social information through in-person, phone and/or virtual visits.
- Develop and implement plan of care per program requirements
- Maintain client records based on best practice, legal and program requirements.
- Maintain operations by following policies and procedures, participating in quality reviews and consultations.
- Ensure operation is in compliance with all applicable federal, state and local rules and requirements.
- Maintain and protect client and record confidentiality by following the Health Insurance Portability and Accountability Act (HIPAA) requirements and Kin On client record policies.
- Maintain good communication with regulatory and/or funding entities (i.e., Department of Social and Health Services, Aging and Disability Services and King County, WA) on program related matters.
- Participate in business meetings and professional development activities
- Monitor and report service contract progress to supervisor.
- Prepare program report as required.
- Collaborate with other Kin On's departments and partner agencies to provide services.
- Perform other duties as assigned Director of Community Wellness and Social Services.
MINIMUM QUALIFICATIONS: - Bachelor's degree in social work, human services or related fields, master's degree in social work or other human service disciplines (e.g., counseling, psychology) preferred.
- Two years of work experience in social or health services.
- Bilingual in English and Chinese preferred.
- Candidate must be authorized to work in the United States.
Knowledge, Skills and Abilities: - Strong knowledge in social work/human services practice
- High level of professionalism, and excellent interpersonal and communications skills (i.e., Able to communicate and establish effective rapport with clients and co-workers)
- Strong project management, organization and problem-solving skills.
- Have a working knowledge of Medicare, Medicaid and public, community resources for older adults in the Washington state and King County, WA.
- Have a valid Washington state driver's license, auto insurance and willing to drive.
- Ability to work collaboratively, independently and respectfully in a socio-culturally and economically diverse setting
Working Environment:
Work is performed in a typical office environment and off-site that may include but not limited to senior centers, public housing, medical setting and client's residence. COMPENSATION AND BENEFITS: - Pay: $25.00/hour - $29.00/hour
