Why This Nursing Role Is Different

You'll continue using your nursing judgment every day while helping patients across observation, inpatient, skilled swing, intermediate swing, and outpatient services receive the right care, at the right time, with the right support.
As our hospital continues to grow, we've expanded this Social Services Advocate & Case Manager role to strengthen care coordination throughout the hospital. You'll help improve communication between patients, providers, insurance companies, community resources, and the care team while helping shape new processes for outpatient infusion services, referral management, discharge planning, and follow-up care.

What You'll Do
As our Social Services Advocate & Case Manager, you will:
Assess patients' clinical, psychosocial, financial, environmental, and discharge planning needs.
Identify barriers related to transportation, caregiver support, housing, insurance, behavioral health, legal concerns, end-of-life planning, and other social determinants of health.
Develop safe discharge plans and coordinate smooth transitions to the most appropriate level of care.
Coordinate referrals for swing bed, skilled care, rehabilitation, home health, hospice, behavioral health, durable medical equipment, and other community resources.
Serve as a trusted resource for patients, families, providers, and staff regarding insurance requirements, discharge options, and available services.
Help patients navigate Medicare, Medicaid, insurance requirements, prior authorizations, and community assistance programs.
Coordinate care planning, family conferences, utilization review activities, and interdisciplinary communication.
Complete psychosocial assessments, cognitive screenings, patient interviews, and documentation.
Advocate for patient rights while supporting informed decision-making and patient-centered care.
Help oversee abuse and neglect investigations while maintaining regulatory compliance.
Support outpatient infusion coordination by improving communication between providers, pharmacy, insurance, and patients so care is delivered without unnecessary delays.
Maintain timely, accurate documentation within the electronic medical record.
Identify opportunities to improve patient flow, care coordination, and the overall patient experience.

Qualifications
Required
Current Montana RN or LPN license with authorization to practice nursing in Montana.
Minimum of two years of nursing or care coordination experience in a hospital, clinic, long-term care, or similar healthcare setting.
Comfortable with technology
RQI Provider certification within seven days of hire, transfer, or before providing independent patient care.
Preferred
Previous experience in case management, discharge planning, utilization review, or social services.
Experience working with community resources, insurance requirements, and post-acute services.
Case Management Certification.

Day Shift - 8 Hours (United States of America)Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed. This position is generally 8a-5p, M-F.
