Overview:

**This position is Hybrid: days on-site/remote will vary based on field needs.**

 

Schedule: Monday-Friday, 8:00 AM-5:00 PM

 

Purpose Statement/Position Summary:  The Social Services Casework is under the supervision of a Master's level social worker, provides specific services to patients and their families. Provides comprehensive, client centered case management services within a Full Service Partnership (FSP) and High Fidelity Wraparound framework, coordinating care across systems of to support individualized, strength-based treatment and community stability. Demonstrates commitment to the principles of family-centered care.

Minimum Qualifications/Work Experience: 1+ years of social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience. Bilingual in English/Spanish preferred.

 

Education/Licensure/Certifications: Bachelor's degree in Social Work or an equivalent combination of related education and work experience. American Heart Association BLS certification required.

Pay Scale Information: CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to, education and experience within the job or the industry. The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates who exceed these specified qualifications and requirements.
