Overview:

**This position is Hybrid: days on-site/remote will vary based on field needs.**

 

Schedule: Monday-Friday, Day Shift

 

Purpose Statement/Position Summary: The Social Services Caseworker position aims to improve, reduce, and eliminate family barriers while advocating, supporting, providing guidance, and coordinating access to care. The Caseworker provides direct care, collaborates with families, multidisciplinary staff, and submits timely documentation to promote children’s social, emotional, and developmental growth. The Caseworker will collaborate with our interdisciplinary Community Behavioral Health team and is responsible for following up with clients/families to assist with community resources such as: Special Education, Regional Center, ABA, Legal Services, Conservatorship, Housing, Transportation, Immigration, Mental Health referrals, Food Banks, and obtaining documentation from community agencies etc. The caseworker is an essential part of a client’s treatment, as it creates opportunities to integrate families into their local community, strengthen partnership with local organizations and increase awareness of available resources.

 

Minimum Qualifications/Work Experience:
Required:

• 1+ years of social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience.
• Lived experience as a parent or caregiver of a child who was hospitalized during infancy and/or received early intervention services during that period.
• Must be willing and able to work fully onsite.

Preferred:

• Bilingual in English/Spanish.
• Knowledge of neurodevelopmental and related disabilities.
• Experience with County and Public Social Services Departments.

Education/Licensure/Certifications: Bachelor’s degree in Social Work or an equivalent combination of related education and work experience. American Heart Association BLS certification required.

Pay Scale Information: CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to, education and experience within the job or the industry. The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates who exceed these specified qualifications and requirements. We provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.
