Social Services Coordinator

Springfield Suites Rehab & Nursing Center is seeking a full-time Social Services Coordinator to join our skilled nursing team.In this role, you will oversee social services, admissions support, and discharge planning while serving as a key resource for residents and their families throughout their stay.Salary: $45,000 per yearResponsibilities:Complete social services assessments, including BIMS, PHQ-9, and trauma screeningsReview admission documentation for appropriateness of placementCoordinate discharge planning and aftercare servicesArrange home care, medical equipment, follow-up appointments, and community resourcesCommunicate with residents and families regarding concerns and care needsCoordinate care plan meetingsMaintain advanced directives and resident recordsDocument assessments and services in PointClickCareQualifications:Experience in social work, admissions, and/or discharge planning in a nursing home or long-term care setting requiredKnowledge of the Medicaid process requiredDegree in Social Services, Human Services, Social Work, or a related field preferredStrong communication, organization, and problem-solving skillsBenefits:Medical InsuranceDental InsuranceVision Insurance401(k)Paid Time OffSupportive team environmentJoin our team and make a meaningful difference in the lives of residents and families every day.
