ABOUT JASC 

JASC was founded in 1946 to assist the approximately 30,000 Japanese Americans resettling in Chicago following their incarceration during WWII. Since then, JASC has evolved into an organization that serves people of all ages, races, and ethnicities from a foundation of Japanese American values.

 

JASC’s programs now focus on social services (home support services, adult day services) and a broad range of educational and cultural offerings for the greater Chicago area. JASC’s publicly accessible, community-based Legacy Center is home to one of the nation’s largest collections on the history of Japanese Americans. In 2022, after decades of growth and recognizing the evolving needs of communities in the Midwest, JASC purchased its “Forever Home” in the West Ridge neighborhood of Chicago.  

 

The Japanese American Service Committee (JASC) is seeking a dedicated, organized, and empathetic Social Services Coordinator to join our team. This role is vital to ensuring the smooth delivery of our support services to clients, particularly seniors and families in need of home care.

The Social Services Coordinator will be responsible for assessing client needs, managing the complex scheduling and communication for our team of homemakers, ensuring meticulous regulatory compliance through reporting and auditing, and directly stepping in to support clients when emergency coverage is needed.

DUTIES & RESPONSIBILITIES

Client & Homemaker Communication

  • Conduct comprehensive initial and ongoing client assessments to determine eligibility, identify specific needs, and develop appropriate care plans.

  • Act as the first point of contact for clients and their families, providing resources, guidance, and support.

  • Oversee and manage the daily, weekly, and monthly schedules for all agency homemakers.

  • Serve as the main hub for communication with homemakers regarding schedule assignments, adjustments, and last-minute rearrangements.

  • Effectively resolve scheduling conflicts to ensure seamless service continuity for clients.

Emergency Response & On-Call Flexibility

  • Maintain the flexibility to respond to early morning and/or weekend calls regarding scheduling emergencies, staff call-offs, or urgent client needs.

  • Escalation & Incident Reporting: Promptly and appropriately escalate complex or critical issues—including critical client needs, medical emergencies, incidents, or unusual occurrences—to a supervisor or manager according to agency protocols.

  • Serve as a backup homemaker: In the event of a staff call-off or sudden shortage, you must be willing and able to step in and directly provide homemaker services to clients to ensure their safety and continuity of care.

Quality Assurance & Compliance

  • Maintain, organize, and update confidential client and employee files in compliance with agency standards and privacy regulations (HIPAA).

  • Conduct rigorous audits of visit logs and service documentation to verify accuracy and accountability.

  • Prepare and submit accurate weekly and monthly operational reports to management and finance teams.

Requirements

  • Minimum of 1–2 years of experience in social services, case management, home care coordination, or a related field.

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field preferred.

  • Proven ability to manage complex schedules, logistics, or shift rotations, preferably in a healthcare or social service setting.

  • Flexibility to respond to early morning, evening, and weekend calls

  • Strong interpersonal and verbal/written communication skills; ability to communicate effectively with a diverse workforce and client base.

  • Exceptional organizational skills and high attention to detail for file management and auditing.

  • Proficiency with Microsoft Office Suite (Word, Excel), Google Suite, and comfort learning specialized scheduling/case management software.

  • Cultural competency or experience working with multicultural communities or older adult populations.

  • Reliable transportation for potential travel to client homes for backup care.
