Key duties include:

• Conducting psychosocial assessments
• Navigating the complex Medicare/Medicaid resources alongside families
• Participating the team care planning process
• Coordinating safe discharge plans
• Resident and family counseling & support
• Regulatory compliance and documentation
• Advocacy and grievance interventions
