Wage Range is $21 to $27 per hour depending on experience
The Social Services Designee assesses resident social service needs and ensures the identified needs are addressed. Advocates for the resident and family, while maintaining a professional and positive customer service approach. As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.
Work Duties
- May support Medical Social Worker or Social worker on a part time basis to manage the uptick in administrative needs.
- Presents and maintains a positive customer service focus when interacting with residents, vendors, visitors and team members;
- Utilize consultation data to assist in planning and coordination of resident care and rehabilitation, following through to ensure service efficacy, modifying plans to comply with changes in condition;
- Ensures that a social service assessment is completed with accurate documentation for all new residents within 5 days in the Health Center or RCFE move-in;
- Assist as needed in discharge planning upon admission for Health Center residents and follows-up with discharge planners for residents who have been admitted to the hospital;
- As required assists in coordinating with the Director of Resident Services / Hospitality / Medical Social Worker and the DON the transfer of residents to appropriate levels of care on campus;
- Ensures that social service is available and provided to residents in the assigned residential location;
- Handles complaints and settling disputes, resolves grievances in coordination with the Social Worker or Administrator. Refers residents and family to community resources when needed;
- Prepares progress notes on residents as needed;
- Documents Social Service issues; charts all social services provided for residents
- Monitor, evaluate and record progress according to measurable goals described in treatment and plan of care;
- Identify and assists residents having problems, i.e. adjusting to placements, behavior problems, etc. through interviews and review of resident records; refer resident to appropriate resources as needed;
- Establishes support groups, as needed, for residents and families;
- Develops and maintains referral files on community resources; and assists residents with selection of appropriate support resources;
- Submits to Administrator or RCFE Director / Manager and provides follow up on all theft and loss as well as complaint reports from residents and family members;
- Attends in-service training workshops and meetings as required to assist in professional growth;
- Works with case managers/discharge planners, physicians, and other organizations to coordinate admissions (if needed) while projecting a professional image of the community;
- Establish involvement with community service organizations for the purpose of outreach;
- Actively promotes community to local hospitals and Assisted Living Communities; participates in marketing;
- Responsible for adhering to HIPAA privacy rules and company policies, procedures and practices which require that resident-identifiable health information will only be used to perform the essential functions of this job;
- Provides effective and courteous service to all residents, guests and coworkers;
- Promotes and protects the rights of each resident;
- Conducts work tasks safely and in compliance with the facility safety program;
- Performs other related duties as required.
