Summary of position

Social Worker position for brain injury subacute rehab program. Provides comprehensive psychosocial support to individuals living with traumatic or acquired brain injuries and their families. The social worker helps clients navigate medical, rehabilitation, emotional, financial, housing, employment, and community-service needs while promoting independence, safety, and quality of life.

Requirements of the position

Required Education and Experience:

• Complete psychosocial assessments and identify client and family needs.
• Develop and coordinate individualized care and discharge plans.
• Educate clients and families about brain injury, available resources, and community services.
• Coordinate referrals to medical, behavioral health, rehabilitation, vocational, housing, and financial-support programs.
• Advocate for clients' rights, needs, and access to appropriate services.
• Assist with benefits, insurance, disability programs, transportation, housing, and other practical needs.
• Collaborate with physicians, nurses, therapists, case managers, neuropsychologists, and other interdisciplinary team members.
• Support clients with adjustment to changes in cognition, behavior, relationships, employment, and independence.
• Facilitate family meetings and provide caregiver support.
• Maintain accurate, timely, and confidential clinical documentation.
• Participate in treatment planning, case conferences, and community outreach activities.
• Complete manager on duty coverage
• Monitor client progress and modify interventions as appropriate.
• Connect clients with long-term brain injury support and community resources.
• Occasionally escort patients to their medical appointments and into the community
• Drive residents into the community with company vehicle

Preferred Education and Experience

• Master's Degree preferred
• Bachelor's degree in social work (SW) or social welfare from an accredited academic institution preferred
• Current state license as a Social Worker preferred
• Experience in outpatient rehabilitation preferred
• Experience in a fast-paced environment
• Post-acute care or health care experience preferred
• Experience working with individuals with brain injuries, disabilities, rehabilitation needs, or complex medical conditions preferred
• Knowledge of brain injury recovery, cognitive impairment, behavioral changes, and family/caregiver dynamics
