Social Services Director (Full-Time)

Diablo Valley Post Acute is seeking a compassionate, organized, and dedicated Social Services Director to join our team. This is an excellent opportunity for a professional who is passionate about advocating for patients and supporting residents and their families throughout their healthcare journey.

Position: Social Services Director

Employment Type: Full-Time

Salary: $80,000-$85,000 per year

Benefits: Comprehensive benefits package available

Responsibilities

  • Assess residents' psychosocial needs and develop individualized care plans.

  • Coordinate discharge planning and transitions of care.

  • Provide emotional support, counseling, and advocacy for residents and families.

  • Collaborate with the interdisciplinary team to ensure quality resident care.

  • Connect residents and families with appropriate community resources and services.

  • Maintain accurate documentation and ensure compliance with state and federal regulations.

  • Participate in care conferences and quality improvement initiatives.

Qualifications

  • Experience in social services within a skilled nursing, post-acute, or healthcare setting preferred.

  • Knowledge of discharge planning, resident rights, and regulatory requirements.

  • Excellent communication, organizational, and interpersonal skills.

  • Ability to work collaboratively with residents, families, physicians, and healthcare professionals.
