Experience with goals of care and end of life planning and conversations, Inpatient Hospital Social Work, Discharge Planning, Oncology. Master s Degree in Social Work (MSW) (required). Licensed Masters Social Worker (LMSW) by the State of New York (required). Communication skills (required). Knowledge of Psychosocial Assessment (required).
