" Must be comfortable coordinating home-care services, equipment, rehabilitation and long-term-care placement
" Crisis intervention and discharge/contingency planning experience are required
" Advance directive experience is required
" Knowledge of Medicaid, Medicare, medical transportation, housing resources and community-based services is required
" Experience with Adult Protective Services, Child Protective Services, guardianship and power of attorney is required
" Experience supporting patients with substance-use concerns and suicide/homicide/self-harm risk is required...
