About the Position

Social Services Program Coordinator III WORK LOCATION: SMRC, Long Beach PAY: $40,286.40 SCHEDULE/SHIFTS: Monday - Friday 8:00 a.m. to 4:30 p.m. RESPONSIBILITIES: Serves as Support Coordinator (SC) for people enrolled in the Home and Community-Based ID/DD Waiver program. Manages and maintains caseload of people receiving ID/DD Waiver services. Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload. Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative. Coordinates and monitors identified supports and services through regular monthly contact, face to face quarterly reviews, and annual assessment and re-certification meetings. Ensures services are adequate, appropriate, and meet the person's needs. Travel throughout the six southern counties will be required. Preferred Qualifications

Typically requires a Bachelor's Degree and 4-7 years of experience. Hiring agency preferred qualifications: Requires a bachelor's degree and 4-7 years of experience and one year of experience working with people with IDD. Please attach your proof of education.
