The Social Services Specialist responsibilities include:
• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community. • Providing therapeutic interventions through assessment, coordination, referral, and interdisciplinary planning. • Being knowledgeable about treatment, social, and economic implications for individuals and families as it pertains to their age, stage of development, and discharge needs. • Possessing a clear understanding of discharge and transition planning and linkage with community resources. • Demonstrating skills in planning, organizing, and managing multiple functions and complex processes.

Certifications and Licensures: Preferred: • ACM (Accredited Case Manager) • CCM (Certified Case Manager)
Education: Required: Masters in Social Work, Sociology, or Related Field Strongly Preferred: Masters Degree in Social Work
Experience: Required: 6 months of Internship or 1 year of Social Work in an acute care or hospital setting
