About the Position
The non-state service, time-limited Social Services Specialist III position will serve as the Community Health Educator for Public Health District II, providing district-level support for community-based chronic disease prevention and health promotion activities under the Office of Adult Health. District II encompasses nine (11) counties: Marshall, Benton, Tippah, Alcorn, Tishomingo, Prentiss, Itawamba, Lee, Pontotoc, Lafayette, and Union, and requires coordinated oversight to address diverse community needs and chronic disease burdens. The Community Health Educator is responsible for supporting outreach efforts and implementation of prevention activities that align with Office of Adult Health priorities. This role provides operational support for delivering evidence-based interventions related to heart disease and stroke prevention, diabetes prevention and control, tobacco control, and other integrated chronic disease strategies at the district level. In addition, the Community Health Educator coordinates evidence-based programs, serves as a professional resource, develops relationships with community members, and engages diverse partners. The position ensures compliance with state and federal reporting requirements, including documentation in Redcap and other required systems to support PHHS Block Grant activities and CDC-funded programs.

The successful applicant must demonstrate strong program coordination and organizational skills, with the ability to manage multiple community-based activities and partnerships across a multi-county region. The position requires knowledge of social service delivery systems, community resources, and chronic disease prevention strategies, including cardiovascular health. The applicant must possess strong communication and relationship-building skills to effectively engage healthcare systems, community-based organizations, and program participants. The role requires the ability to support referral tracking and follow-up, maintain accurate documentation, and contribute to data collection and reporting efforts. Proficiency with Microsoft Office and data tracking systems is required, along with the ability to work independently, exercise sound judgment, and adapt to evolving program needs while ensuring compliance with state and federal requirements.
