Our guiding motto is leading with integrity, empowering people, delivering outcomes.Job Description:The Social Services Supervisor is responsible for assisting residents to manage their everyday emotional, mental, and psychosocial needs: Advocate for resident needs and rights Maintains contact with family, in consultation with the resident, to report on changes in health, goals, discharge planningMakes referrals and obtain services from outside entities, e.g., talking books, absentee ballots, etc.Assists residents with financial and legal matters, e.g., Social Security, VA, referrals to funeral homes for preplanning arrangementsBuilds relationships between residents and staff and teaches staff how to understand and support resident's individual needs.
