Summary:

Healthy Start is seeking a Care Coordinator to provide comprehensive, on-site education and support to pregnant and parenting individuals and their families. The Care Coordinator will focus on assessing social determinants of health (SDOH), connecting families to resources, coordinating services, and providing education that promotes healthy pregnancies, safe parenting, and improved maternal and infant outcomes.

Essential Responsibilities:

  • Screen and assess families referred to the Healthy Start program to determine individual needs
  • Complete or attempt to complete at least one monthly face-to-face visit with each participant
  • Complete or attempt to complete at least one telephone contact monthly to maintain engagement
  • Develop Individualized Plans of Care and Family Support Plans for each client that clearly identifies specific goals, objectives and service needs
  • Facilitate access to community resources and follow up
  • Reschedules missed Healthy Start appointments
  • Timely and accurately complete progress notes
  • Completes all mandatory trainings as required by the program, the funder, and the agency
  • Meets grant goals and objectives, programmatic and funder requirements
  • Maintains a case load according to program requirements
  • Participate in community outreach events as needed
  • Comply with mandatory reporting requirements for suspected child abuse or neglect and follow agency policies on risk and safety

Education:

  • Bachelor's degree in social sciences, psychology, health care administration, or social work.

Experience:

  • Proficiency in Microsoft Office (Ex. Word, Excel, Outlook, PowerPoint)
  • Minimum of 2 years of professional experience working in the community or social services, Preferred
  • Bilingual in English and Spanish, Preferred
  • Case Management, Mental Health, or Nursing work experience

Preferred Qualifications:

  • Experience working with perinatal populations, child welfare-involved families, or home visiting programs or early childhood services.
  • Familiarity with local maternal-child health resources, behavioral health providers, and social service systems.
  • Must have reliable transportation; possess a valid Driver License and auto insurance coverage.
  • Requires flexibility, independent time management, and comfort working with families in varied home environments.

Work Environment:

  • Provides services at Broward Health facilities that provide onsite pregnancy or birthing healthcare services
  • Conducts home visits or other safe location on an as needed basis
  • General office conditions is the standard work environment (standing, sitting, and walking)

Essential:

  • Basic Life Support within 30 days of hire

  • AHCA Level II - Required
