Social Services Worker

Desired Qualifications
- Licensed Social Worker in the State of Alabama preferred
- Degree or educational background in Social Work, Social Sciences, or a related field
- Previous long-term care or skilled nursing experience preferred
- Strong documentation and organizational skills
- Excellent communication and interpersonal skills
- Ability to effectively follow up with residents, families, staff, and outside providers
- Compassionate and professional approach when working with residents and families
Essential Job Functions
The Social Services Worker serves as an advocate for our residents and helps ensure their medically related emotional, social, and psychosocial needs are identified and addressed.
Responsibilities include:
- Plan, organize, implement, and evaluate the facility’s Social Services program
- Advocate for residents and serve as a liaison between residents, families, staff, and outside resources
- Identify and address residents’ social, emotional, and psychosocial needs
- Assist residents and families with adjustment to long-term care and changes in health or level of care
- Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
- Participate in resident care planning and interdisciplinary team meetings
- Assist with discharge planning and coordination of community resources when applicable
- Communicate regularly with residents and families regarding needs, concerns, and available resources
- Ensure Social Services are provided in accordance with applicable federal, state, and local regulations
- Work closely with facility leadership and the interdisciplinary team to promote each resident’s overall quality of life
