Social Work Assistant

Responsibilities:

• Assist licensed social worker in assessing client needs and developing care plans
• Provide direct support and resources to clients and their families
• Maintain accurate and up-to-date client case files and documentation
• Coordinate referrals to community resources, programs, and support services
• Conduct follow-up calls and check-ins to monitor client progress and well-being
• Collaborate with multidisciplinary teams to ensure comprehensive client care
• Assist in facilitating group sessions, workshops, or community outreach programs

Requirements:

• Bachelor's degree in Social Work and other  Social Sciences.
• Prior experience in a social services, case management, or community support role is a plus
• Strong interpersonal and communication skills with a client-centered approach
• Ability to maintain confidentiality and handle sensitive information with professionalism
• Proficiency in basic computer applications and case management software
• Bilingual skills (English/Spanish or other languages) a plus
• Valid driver's license and reliable transportation is required
