Schedule: 20 hours per weekMonday - Friday between the hours of 8:00 am - 4:00 pm | No nights, weekends, or holidays!

Location: St. Agnes Hospital - 430 E. Division St. Fond du Lac, WI

This position is dedicated to connecting underserved individuals in our community with local resources and support services.

Bachelor's Degree required. Candidates with a background in non-profit work are encouraged to apply!

This role is part-time benefits eligible.

Job Summary:

Investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

• Interviews patients, assesses priorities and documents case activity.
• Consults with case managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
• Assists clients/families with obtaining community assistance by referral to proper resources.
• Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
• Supports management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.

EDUCATION

• Bachelor's degree in Health Care or related field

EXPERIENCE

• One year experience

PHYSICAL REQUIREMENTS

• Frequent reaching, sitting, standing and walking.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
• Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
• Frequent keyboard use/data entry.
• Occasional bending, stooping, climbing, twisting and gripping.
• Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Occasional use of vision to identify and distinguish colors.
• Occasional driving.
• Rare repetitive foot/leg and hand/arm movements and squatting.
