UNIT- MICU
13 weeks, full time, M-F 8:30- 5 p.m
Requirements: - MD LMSW or LCSW-C
- Inpatient critical care experience required
- Must have recent hospital and discharge planning experience
- EPIC experience
Job Description: Early identification, high-risk screening, and readmission screening of all patients. Services include, but are not limited to, adjusting to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge.
(Candidates may work with an active, out-of-state license that is equivalent to the Maryland license. A completed application must be on file with the board for clearance to work)
