Key Responsibilities: Lead and manage Social Work Supervisor, Social Workers, Behavioral Health Specialists, and Social Work Assistants. Handle hiring, training, supervising, and coaching of staff. Oversee quality of social work assessments and IDT (Interdisciplinary Team) participation. Ensure timely and effective delivery of social work interventions based on participant care plans and urgent needs. Maintain compliance with documentation, assessments, care plan completion, reviews, and participant rights. Collaborate with Behavioral Health Specialists to manage programs such as support groups, counseling, and family education. Partner with community professionals when needed. Provide support in end-of-life care coordination. Conduct home visits as required. Job Requirements: Master's degree in Social Work ( MSW) required. Social Work licensure preferred. At least 2 years of supervisory or leadership experience in social work strongly preferred. Minimum 1 year of experience working with frail or elderly populations. Strong clinical, organizational, and communication skills. Valid driver's license with clean DMV record. Proof of current vehicle insurance required.
