Job Title: Social Worker (MSW)

Shift Details:
Flexible 8-hour and 10-hour shifts available
- 8-hour shifts: 8:00 AM 4:30 PM or 3:00 PM 11:30 PM (ED support)
- 10-hour shifts: 3:00 PM 1:30 AM (ED support)
- Weekend rotation required
- No holiday requirement
Job Summary
We are seeking an experienced and compassionate Master's-level Social Worker (MSW) to provide comprehensive social work services within a healthcare setting. This role involves supporting pediatric and adolescent populations, collaborating with families and interdisciplinary teams, and facilitating effective care plans. Candidates must be highly adaptable, as minimal onsite training will be provided.
Key Responsibilities
- Deliver direct social work services to patients and families, addressing psychosocial and environmental needs
- Develop and implement individualized care plans using the social work process
- Collaborate with healthcare providers, school staff, and families to ensure optimal patient outcomes
- Support discharge planning and care coordination
- Provide referrals to community resources and external agencies
- Maintain clear and effective communication with patients, families, and team members
- Ensure efficient utilization of resources while maintaining quality care standards
Required Qualifications
- Master's Degree in Social Work (MSW) from an accredited program
- Active Michigan Social Work License: LMSW or LLMSW
- Minimum 5+ years of case management experience
- At least 2+ years of recent acute inpatient case management experience (REQUIRED)
- Strong experience in discharge planning
- Knowledge of pediatric and adolescent patient care
- Valid Michigan driver's license and insured vehicle
Important Notes
- Recent inpatient case management experience is mandatory
- Experience in telephonic case management, utilization review, or purely psychological roles will not be considered
- Limited onsite training candidates must be able to ramp up quickly
