Position Description
POSITION DESCRIPTION
Harris County Precinct 4 seeks Social Work Navigators to join the Social Services Department. This role supports the Operations Division by bridging gaps between frontline services and policy opportunities, while contributing to resource-related projects aligned with the agencys mission as well as providing direct services to constituents in need.
This position requires hands-on experience in case management, advocacy, resource navigation, and macro-level social work initiatives. Navigators will engage in direct client interactions, research, project management, and fieldwork while following a client-centered, holistic approach. Under supervision from a social worker field supervisor, and licensed therapist, the navigator will develop professional competencies and receive guidance to enhance their professional growth in service of the Precinct 4 community.
Duties and Responsibilities:
Case Management and Client Services
Provide case management services, including assessments, service planning, and follow-up.
Communicate with clients via phone, email, virtual platforms, and in person.
Use technology systems for client information, case tracking, and documentation.
Utilize motivational interviewing and trauma-informed approaches to assess client needs.
Ensure accurate documentation and maintain client confidentiality.
An ability to provide and perform essential staff responsibilities including emergency case management and support during disaster relief efforts in person as a integral part of the emergency response team (e.g. weather events, warming/cooling center or temporary shelter support, food distribution support, mobile crisis support, and any other duties needed in the time of a disaster.)
Advocacy and Research
Identify gaps between frontline service delivery and policy implementation.
Develop recommendations to improve organizational practices and promote equitable access.
Advocate, Research and prepare materials for advocacy initiatives addressing community needs and on behalf of constituents on a path to empowerment, access and knowledge of resources.
Support commissioners court social service needs and program.
Resource Navigation and Workshop Development
Create and facilitate social service and program workshops and events.
Collaborate with agency staff to maintain and update community resource guides.
Strengthen resource networks by coordinating with community partners.
Fieldwork and Community Engagement
Travel to community centers, outreach events, agencies, and client locations to deliver services.
Represent the agency at community meetings, resource fairs, and advocacy events.
Administrative Duties
Manage and resolve client service tickets in a time efficient manner.
Participate in team meetings, training sessions, and supervision.
Utilize data and case management systems for reporting and decision-making.
Assist with special projects and adhere to agency policies and procedures.
Perform other duties as assigned by the direct supervisor and Social Services Director.

Requirements
Education:
Master's degree in social work (MSW) or related master's program from an accredited U.S. program.
Flexibility to travel locally to community centers, agencies, and outreach locations.
Adherence to social work values, ethics, and professional principles or related field board and ethics standards.
Knowledge, Skills, and Abilities:
Demonstrated empathy, patience, and inclusiveness.
Strong organizational, listening, and problem-solving skills.
Excellent verbal and written communication abilities.
Ability to manage competing priorities and adapt to change.
Proficiency in technology, including case management software, ServiceNow, Apricot/Banterra, and Microsoft Suite.
Ability to work independently and as part of a team while maintaining confidentiality.

Preferences
Bilingual (e.g., Spanish, Arabic, Pashtu/Dari, Vietnamese, Chinese, Amharic, Oromo, Tigrigna, etc.).
Knowledge of local, state, and federal social service delivery systems.
Mental Health First Aid or Motivational Interviewing certification.
LMSW, LCSW, LMSW-S, LCSW-S, LPC-A, LPC, LPC-S to practice in Texas, or in the process of acquiring licensure.
Three (3) years of experience addressing constituent concerns, resource navigation, case management, or social service support in the field.
Exceptional listening and problem-solving skills.
Exceptional conflict resolution skills and crisis stabilization support
Strong interpersonal, communication and organizational skills.
Experience working with Social Work graduate level university internship programs or related graduate level internship programs.
Experience with Disaster Relief Efforts
Ability to effectively interact with diverse groups, with dignity and respect.
